How do I request a proposal?
You can request a proposal by contacting a Davis Vision sales representative. Use the “Contact a Sales Representative” tool above. Select your state from the drop-down menu to receive the name and toll-free phone number of your Davis Vision representative.
How long does it take to obtain a proposal?
The turnaround for a standard proposal is three to five business days. However, we can accommodate tighter deadlines when needed. Turnaround for full-scale RFP response that includes questions and answers is based on the due date specified within the RFP.
What information is required for a proposal request?
In addition to the group name, contact information, size, and situs state, the following information will help us further customize our proposal to meet your needs:
- Group’s current plan design specs and plan design specs to be quoted. Including benefit frequency, allowances, co-payments, in network and out of network coverage.
- Participation for current plan (voluntary or employer paid)
- Participation requested for quote (voluntary or employer paid)
- Funding for current plan (ASO or insured)
- Funding requested for quote (ASO or insured)
- Define populations to be covered/eligible (retirees/actives). Note whether there will be any difference in plan design, participation or funding for each population.
- Census data and ZIP codes of eligible employees
- Current rates/rates history
- Paid claims and cost of administration
- Number of paid claims & membership history (ee’s & dependents)
- Usage by service (i.e., # exams, # frames, # lenses etc.)
- Commission (if applicable)
- Number of employees enrolled in the program
- Number of employees eligible to enroll
- Number of employees that used the benefit
- Number of employees by tier (e.g. how many enrolled in Employee Only vs. Family coverage, etc.)
- Total number of lives covered (employee and dependent counts combined and separately)
- How long has current vendor held vision contract? Prior vendors? When vision plan was first implemented.
Group Qualification
- The minimum requirement for funded, employer-paid plans is 10 enrolled employees (contracts).
- The minimum requirement for voluntary, contributory/employee-paid plans is 50 eligible employees with 10 enrolled.

Composite
Applicable when the employer desires a per-person rate for each individual covered. The composite rate is to be used for each employee, regardless of family size.
Effective Date
Davis Vision benefit plans are effective on the first of the month. To guarantee proper implementation, Davis Vision requires thirty-day advance notice for account installation. All enrollment requirements must be met before the effective date.
Can members receive care from the provider of their choice?
Most all Davis Vision plans offer members the freedom to visit any provider of their choice. Of course, members receive the greatest benefit value when they visit one of our 30,000+ network providers, where many plans offer paid-in-full coverage. Some groups prefer to offer “in-network” benefits only.
Do you offer individual vision coverage?
Davis Vision programs are designed as true group products, precluding individual products.
