How can I find a Davis Vision provider?
If you are a registered user for Benefit Manager™, log in and use our “Find a Provider” search tool. If you are a current Davis Vision client and do not use Benefit Manager, you may locate a provider by logging in as a member. If you are not currently a Davis Vision client, you may request access to review our provider network by contacting a Davis Vision sales representative. Use the “Contact a Sales Representative” tool above. Just select your state from the drop-down menu to find the name and toll-free phone number of the Davis Vision representative in your area.

How can I sign up to use Benefit Manager?
Just contact your Davis Vision Client Manager. If you do not know the contact information for your Client Manager, simply fill out this form and a Davis Vision client manager will get in touch with you shortly.

How can I sign up to receive my invoice electronically (e-Bill)?
Please fill out this form and fax it to 1-800-646-5824. If you have questions concerning e-Bill or the completion of this form, please call
1-877-393-7801 and choose option 1.

Can members receive care from the provider of their choice?
Most all Davis Vision plans offer both in-network and out-of-network benefits, allowing members the freedom to visit any provider of their choice. Of course, members receive the greatest benefit value when they visit one of our 30,000+ in-network providers, where many plans offer paid-in-full coverage. Some groups prefer to offer “in-network” benefits only.

Do members need a claim form for services?
When visiting a Davis Vision participating provider, there is no need for a claim form. A member simply provides his or her ID number when making an appointment. The provider verifies eligibility and benefits and handles the rest. If your group’s vision benefit includes an “out-of network” benefit members may visit out-of-network providers. In this instance, they will be responsible to pay the out-of-network provider’s charges at the time of service, and then submit a claim form for reimbursement. Claim forms are available on our Web site.

Do you offer online tools for benefits administrators?
Yes! Davis Vision’s offers a number of online capabilities that greatly assists with benefit administration. With Davis Vision’s Benefit Manager, groups have the ability to locate providers, access forms including benefit booklets and out-of-network claim forms, check enrollment and eligibility for members and access additional vision information. For groups that do not use electronic enrollment, Benefit Manager can also be used for adds, changes and terminations to enrollment information.

Groups can also enjoy the convenience of e-Bill, a secure online system that offers the ability to pay invoices, view past and current invoices/payments, and export the data into Microsoft Excel for your reporting purposes.
