What information do I need to submit with the RFP?
To issue a proposal, Davis Vision will need the following information:

• Group name and address
• Number of employees
• Employee zip code listing
• Current census data, including number of contract holders and their dependents
• Waiting period for new employees
• Current or proposed vision plan design information, including in-force benefits
• Benefit utilization history, if available
