a Proposal
The following prerequisites should be noted when requesting group
proposals:
Underwriting Requirements
Davis Vision programs are designed for pure groups only,
precluding individual enrollment, and rates are charged on a per-month
basis. Large groups rates are guaranteed for two years from
the plans effective date.
Group Qualification
Fully insured and self-funded, non-contributory programs require
a minimum of 300 full-time employees. The sponsor pays 100% of the
premium for all beneficiaries and dependents. Information on the
exclusion of some beneficiaries and dependents should be submitted
in advance for Davis Visions approval.
Groups wanting beneficiaries to pay a portion of the premium must
receive approval from Davis Vision prior to quoting rates. Voluntary
offerings require a minimum of 1,000 eligible employees.
Group Information
Please provide the following to enable us to quickly develop the
most accurate proposal possible:
- Group name and address
- Number of employees
- Employee zip code listing
- Current census data, including number of contract holders and their dependents
- Waiting period for new employees
- Current vision plan information, including in-force benefits
- Benefit utilization history, if available
Rates and Regional Variation
Davis Vision assigns each state to a regional pricing zone. All
quotes must reflect the regional variation. However, if a specific
groups demographics (for multi-state sponsors) have been presented,
a case-specific quote will be developed.

Rates can be presented in the following formats with the categories
mirroring the company's Major Medical Coverage:

Composite
Applicable when the employer desires a per-person rate for each
individual covered. The composite rate is to be used for each employee,
regardless of family size.
Contract
Davis Visions agreement/policy will be prepared for the client
groups signature upon receipt of all enrollment requirements.
Also, you will receive a commission agreement. Commissions will
be paid on a monthly basis by Davis Vision following the effective
date and upon receipt of the signed contract and benefit consultant agreement.
Effective Date
Davis Vision benefit plans are effective on the first of the month.
To guarantee proper implementation, Davis Vision must receive at
least a thirty-day advance notice. All enrollment requirements must
be met before the effective date.
